In connection with the online health consultation service provided by WM Health Istanbul ("WM Health Istanbul"), I confirm that I have read the Privacy Notice and freely give my explicit consent to the following.

1. Subject of Consent

I explicitly consent to the health data I share when booking and that arises during the consultation being:

• shared with the doctor who will conduct the consultation, for the pre-assessment and preparation of the consultation,
• stored in my account and sent to me, in the form of notes and recommendations prepared by my doctor after the consultation,
• shared with a doctor in Türkiye, if I am receiving the service from abroad, so that the service can be provided.

2. Special Category Data to Be Processed

• Complaint and medical history,
• Answers in the pre-assessment form,
• Test results, prescriptions, reports and imaging documents I upload,
• Health information I share during the consultation and the doctor's notes.

3. Consultation Recording Policy and Consent

Video consultations are not recorded by default. If the doctor wishes to record during the consultation, recording starts only if I give separate consent on screen. A recording indicator is shown on screen while recording. Recordings are used only for medical follow-up and complaint investigation, are not shared with unauthorised persons and are automatically deleted after 30 days. Refusing to consent to recording does not prevent me from receiving the service.

4. Quality Control of Consultations

I have been informed in Section 7 of the Privacy Notice that consultations may be joined, as a listener only, by an authorised WM Health staff member bound by confidentiality for quality control purposes, that a notice is shown on screen when this happens, and that listening does not create a recording.

5. Withdrawal of Consent

I may withdraw this consent at any time by writing to info@wmhealthistanbul.com. Withdrawal does not affect the lawfulness of processing carried out before the withdrawal. If I withdraw my consent, services that require processing of my health data may no longer be available to me.
